Case Aide at The Epiphany Center, San Francisco

SUMMARY:
Under the supervision of the Program Director, the Case Aide is responsible for case support to
the Case Managers and all data entry.

QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform the essential job duties
and meet the qualifications listed below:
1. A Bachelor’s degree in a Behavioral Science preferred.
2. Good verbal and written communication skills.
3. Requires a valid CA Driver's License and a good driving record.
4. Must possess or be able to obtain current CPR and First Aid certificates.
5. Must have computer skills in word processing and data entry programs.
6. Must meet agency conditions for employment including clearance for Child Abuse.

ESSENTIAL DUTIES:
1. Works with the Program Director to provide case aide support to the Case Managers in
doing outreach to potential clients and in making referrals to other agencies for services
outlined in client treatment plans.

2. 1:1 contact with clients providing support at Medicare appointments, life skill
appointments as well as contacting medical professionals.

3. Maintains client files to meet all compliance requirements.

4. Completes all data entry as required in an accurate and timely manner to meet City, State,
and Federal requirements.

5. Maintains resource files, keeps forms and information packets current.

6. Provides transportation for clients in agency vehicles as requested by the Program
Director.

7. Provides support for clients in enrolling in vocational, GED, or college classes.

8. Maintains client confidentiality in all circumstances as required by professional ethics and
legal requirements.

9. Maintains any calendar, attendance, and sign-in records as required.

10. Communicates effectively with coworkers, clients and personnel associated with outside
agencies.

11. Performs effectively and professionally under pressure.

12. Arrives to work and meetings promptly.

13. Acknowledges safety policies and procedures and reports potentially unsafe conditions.

14. Other tasks as assigned.
